If you've just started Mounjaro or Wegovy, you might be wondering whether you actually need to exercise on Mounjaro or Wegovy, or whether the medication is doing enough on its own. The honest answer is that you can, and should, stay active alongside either treatment. It will not stop the medication from working, but exercise plays a bigger part in your results than most people realise.
This guide covers how much exercise you actually need, which type matters most, whether it's safe to exercise around dose increases, and what tends to happen to your body if you skip it altogether. If you're still deciding between treatments, our oral semaglutide guide covers how Wegovy's tablet form compares to the injectable versions discussed here.
Can you exercise on Mounjaro or Wegovy?
Yes. Exercise is safe alongside both treatments and does not interfere with how they work. Mounjaro and Wegovy act on appetite and digestion rather than on your muscles or cardiovascular system directly, so physical activity and the medication simply work alongside each other. Both treatments are licensed to be used together with a reduced calorie diet and increased physical activity, not as a stand-alone solution.1,2
The one thing worth planning around is how you feel in the first few weeks. Nausea and tiredness are very common while your body adjusts to Wegovy, and fatigue is a common side effect of Mounjaro.1,2 Neither is a reason to avoid exercise altogether. It's usually easier to lower the intensity for a week or two than to stop being active completely.
Will exercise make Mounjaro or Wegovy side effects worse?
For most people, no. Gentle activity such as walking can actually help with digestion and often eases mild nausea rather than worsening it. The exception is the few days following a dose increase, when gastrointestinal side effects tend to be at their strongest (for both treatments) as your body adjusts to the higher dose.1,2
There is no need to stop exercising altogether around a dose change. Reducing the intensity of your sessions for a few days, staying well hydrated, and paying attention to how you feel is usually enough to get through it comfortably. If nausea or fatigue are severe enough to stop you exercising for more than a week or two, mention this to your prescriber, since it may be worth reviewing how quickly your dose is being increased.
How soon after starting Mounjaro or Wegovy should you start exercising?
There's no need to wait. You can start or continue your normal exercise routine from your very first dose, as long as you feel well enough to do so. Some people prefer to ease into a new routine during the first week or two while nausea and appetite changes settle, which is a reasonable approach, but there's no clinical reason to delay starting altogether.
If you weren't exercising much before starting treatment, the first few weeks are actually a sensible time to begin building the habit. Appetite is often lower in the early stages, so energy for intense exercise may be reduced, but lighter activity such as walking is usually well tolerated from day one.
Is it safe to exercise on an empty stomach on Mounjaro or Wegovy?
Both medications reduce appetite, so it's common to go longer between meals than you're used to. Exercising in a fasted state is generally fine for light to moderate activity such as walking, but it's worth being cautious with more intense sessions, particularly if you also take a diabetes medication such as insulin or a sulfonylurea alongside Mounjaro or Wegovy, since the combination can increase the risk of low blood sugar during exercise.1,2
A small snack containing some carbohydrate and protein, such as a banana with a spoonful of nut butter, about 30 to 60 minutes before a more demanding session is a reasonable precaution if you feel lightheaded, shaky, or unusually fatigued during exercise on an empty stomach.
Do you need to exercise while taking Mounjaro or Wegovy?
Strictly speaking, you'll likely still lose weight on Mounjaro or Wegovy without doing any exercise, because both medications work mainly by reducing how much you eat. What exercise adds is greater control over what kind of weight you lose.
Rapid weight loss from a calorie deficit alone costs some muscle as well as fat. In the SURMOUNT-1 trial of tirzepatide (Mounjaro's active ingredient), around a quarter of the weight lost was lean mass - similar to the placebo group losing weight through diet and lifestyle alone.3
That figure is a trial-wide average, though, and it hides variation between individuals. People who stay very inactive during treatment tend to lose a larger share of muscle than those who keep moving. So exercise still matters, even though the headline number looks manageable. You can read more on this, including who's most at risk, in our guide on whether Mounjaro or Wegovy causes muscle loss.
Does exercise affect how well Mounjaro or Wegovy works?
Exercise doesn't change how semaglutide or tirzepatide work in your body. Both medications act on GLP-1 and, in Mounjaro's case, GIP receptors, regardless of your activity levels.1,2 What exercise changes is the outcome you get from that weight loss, not the underlying mechanism of the drug.
It's worth knowing that the headline weight loss figures usually quoted for these treatments, around 15% of body weight for Wegovy's standard maintenance dose and up to 22.5% for Mounjaro's highest dose, both come from trials in which participants also received structured dietary and lifestyle counselling as part of standard care.4,5 The medication was never tested in isolation from lifestyle support. That's one reason real-world results can fall short of trial results when the lifestyle side of treatment is left out.
How much exercise do you need on Mounjaro or Wegovy?
The standard NHS guidance for adults applies here, and there's no need to do more than this just because you are on treatment:6
- At least 150 minutes of moderate intensity activity a week, or 75 minutes of vigorous intensity activity, or a mix of both
- Strengthening activities that work the major muscle groups on at least 2 days a week
A simple way to judge intensity without any equipment is the talk test. During moderate activity, such as a brisk walk, you should be able to hold a conversation but not sing. During vigorous activity, such as running, you will only manage a few words at a time before needing to pause for breath.6
If 150 minutes sounds like a lot to start with, it's fine to build up gradually. Someone who has not exercised in a while doesn't need to hit that figure in week one. Two short walks and one light strength session in your first week is a reasonable starting point, with the aim of building towards the full guidance over the following months.
What's the best type of exercise on Mounjaro or Wegovy?
Most of the benefit comes from combining two types of activity rather than relying on one:
- Resistance training - lifting weights, using resistance bands, or bodyweight exercises like squats and push-ups. This is what protects muscle mass while you're in a calorie deficit, and it's the type of exercise people on GLP-1 medication most often skip.
- Cardiovascular exercise - walking, cycling, swimming, or anything that raises your heart rate for a sustained period. This supports heart health and blood sugar control, and both Mounjaro and Wegovy have been shown to improve cardiovascular risk factors when combined with an active lifestyle.3,4
If you can only manage one to begin with, resistance training generally has the bigger impact on how your body composition changes, though both matter for overall health.
A simple starting point for each:
- Resistance training examples - bodyweight squats, push-ups, resistance band rows, or a pair of light dumbbells for exercises like bicep curls and shoulder presses. None of this requires a gym membership to begin with.
- Cardio examples - a brisk 20 to 30 minute walk, a short cycle, or a few lengths of a pool. Low-impact options are worth prioritising if you're carrying extra weight or have joint discomfort, since they're easier to sustain consistently.
What if you can't exercise much yet?
If you're starting from a lower level of fitness, carrying a significant amount of excess weight, or managing joint pain, the guidance above is a target to build towards, not a condition you need to meet before starting treatment.
Something is almost always better than nothing. A few minutes of standing and moving around during the day, seated resistance exercises, or a short walk to the end of the road all count as a starting point. Many people find that as weight loss progresses on Mounjaro or Wegovy, joint pain and breathlessness ease, which makes building up activity more manageable over the following months rather than something you need to force from week one.
If a health condition makes you unsure what is safe, a GP or physiotherapist can help you find a starting point suited to you.
How to tell if your exercise routine is working
The scale alone doesn't tell the full story, especially in the first few months. Weight can stay the same or move slowly even while your body composition is genuinely improving, because muscle is denser than fat and takes up less space for the same weight.
Better markers to track alongside the scale include:
- How your clothes fit, particularly around the waist
- Whether everyday activities like stairs or carrying shopping feel easier
- Whether you can lift slightly more, do slightly more reps, or walk slightly further than when you started
- How you feel day to day, including energy levels and sleep quality
If several of these are improving even when the number on the scale isn't moving much, your routine is very likely doing its job.
What happens if you don't exercise on Mounjaro or Wegovy?
You'll most likely still lose weight, since the appetite reducing effect of both medications continues regardless of your activity levels. What tends to change is the composition of that weight loss and how you feel day to day.
Without resistance training in particular, a larger share of the weight lost is likely to come from muscle rather than fat. Over time this can mean feeling weaker, having less energy for daily tasks, and finding it harder to maintain your results if you eventually reduce or stop treatment. When should you seek medical advice?
Speak to a GP or your prescriber if:
- You feel persistently dizzy, faint, or short of breath during light activity
- You have chest pain during or after exercise
- You cannot keep fluids down and are showing signs of dehydration
- You have an existing heart condition, joint problem, or other condition that affects what exercise is safe for you
Frequently Asked Questions
Can you lose weight on Mounjaro without exercising?
Yes. Both medications reduce appetite enough to cause weight loss through diet alone. Without exercise,
particularly resistance training, a larger share of that weight loss is likely to come from muscle rather
than fat, which is why staying active is recommended rather than optional.
Do you have to exercise every day on Mounjaro or Wegovy?
No. NHS guidance recommends at least 150 minutes of moderate activity spread across the week, plus 2
strength sessions, not daily exercise. Spreading activity across 4 to 5 days with rest days in between is a
sustainable approach for most people.
What's the best exercise for weight loss on Mounjaro?
There isn't a single best exercise. Combining resistance training, such as bodyweight squats or light
weights, with regular cardio, such as walking or cycling, gives the best results by protecting muscle mass
while supporting heart health and blood sugar control.
Can you still exercise on Mounjaro if you have joint pain?
Yes, though low-impact options such as swimming, cycling, or walking are usually more comfortable than
high-impact activities like running. Joint pain often eases as weight loss progresses, making it easier to
build up to more varied exercise over time.
Should you exercise on Mounjaro if you feel nauseous?
Light activity such as walking is usually fine and can sometimes ease mild nausea. It's best to save
more intense sessions for when you feel better, particularly in the days following a dose increase, when
nausea tends to be at its strongest.
How much exercise do you need each week on Wegovy?
The same as general NHS guidance for all adults: at least 150 minutes of moderate activity, or 75
minutes of vigorous activity, plus muscle-strengthening exercise on at least 2 days a week. There's no
additional requirement specific to Wegovy.
