Unlike the Wegovy injection, Wegovy semaglutide tablets come with a set of precise dosing rules. Take it on an empty stomach, use only a small amount of water, and wait 30 minutes before eating or drinking anything else.
These aren't random restrictions. They're built into the science of how the tablet works.
This guide walks through the full Wegovy tablet titration schedule, explains why the fasting window matters, and covers what to do if you miss a dose or struggle with side effects at a particular stage. Oral semaglutide received MHRA approval on 11 June 2026.
Empty stomach, a small sip of water, then 30 minutes before anything else. That routine is what makes the tablet work.
What doses does the Wegovy tablet come in?
Wegovy semaglutide tablets come in four strengths: 1.5mg, 4mg, 9mg and 25mg. These match the four stages of the titration schedule.
You don't start at 25mg. The dose goes up gradually over 90 days, giving your body time to adjust to semaglutide before you reach the full maintenance dose.
Why do the tablet doses look so much higher than the injection, which tops out at 2.4mg a week? It's all about absorption. Less than 1% of each oral dose reaches your bloodstream.1 The much bigger tablet dose makes up for what's lost in your digestive system, so enough semaglutide reaches your circulation to work.
What is the Wegovy tablet titration schedule?
The Wegovy tablet dose goes up every 30 days, and most people reach the full 25mg maintenance dose from day 91.2
| Phase |
Dose |
Duration |
| Starting dose |
1.5mg once daily |
Days 1 to 30 |
| First escalation |
4mg once daily |
Days 31 to 60 |
| Second escalation |
9mg once daily |
Days 61 to 90 |
| Maintenance dose |
25mg once daily |
Day 91 onwards |
Each increase should only happen once you're coping well with the previous dose. Getting significant side effects at any stage, especially nausea, vomiting or diarrhoea? Your prescriber may suggest staying on your current dose for longer before moving up.
Skipping ahead isn't a good idea. The titration exists specifically to reduce side effects, and going up faster doesn't improve weight loss.
Do not move up a dose early. A slower titration means fewer side effects, not less weight loss.
How do you take Wegovy tablets correctly?
According to the prescribing information, you should take Wegovy semaglutide tablets:2
- Once a day in the morning - on an empty stomach
- With water only - up to 120ml (about 4 ounces), and no other drink
- Whole - swallowed without splitting, crushing, chewing or dissolving the tablet
- At least 30 minutes before - eating, drinking anything other than water, or taking any other oral medicine
These aren't just general tips. Each one is directly linked to how the tablet is absorbed in your stomach, and not following them can significantly reduce how much semaglutide gets into your bloodstream.
Why does the Wegovy tablet have to be taken on an empty stomach?
This is the question most people have when they first see the dosing instructions. The answer comes down to the absorption technology built into the tablet.
Oral semaglutide uses an ingredient called SNAC (salcaprozate sodium). It makes it possible for semaglutide, a large protein molecule, to survive your stomach and reach your bloodstream. On its own, semaglutide would quickly be broken down by stomach acid and digestive enzymes before it could be absorbed.
SNAC protects semaglutide in three ways at once:3
- pH buffering - SNAC creates a small area of higher pH right where the tablet touches your stomach lining. This temporarily neutralises the acid and switches off pepsin, the enzyme that would otherwise break semaglutide down.
- Breaking up clusters - semaglutide naturally clumps together in solution. SNAC breaks these clumps into single molecules (monomers), which are small enough to cross your stomach wall.
- Opening the stomach lining - SNAC temporarily makes the cells of your stomach lining more permeable, so semaglutide can pass through into your bloodstream.
Food gets in the way of all three. With food in your stomach, the SNAC around the tablet is diluted and the protective environment it needs is disrupted. The result is much less semaglutide absorbed, and a weaker effect from your dose.
The same goes for the water limit. Taking the tablet with more than about 120ml of water dilutes the SNAC where it needs to work.3 Use a small glass, not a full tumbler.
What happens if you eat before taking a Wegovy tablet?
If you take your tablet with food in your stomach, or eat within 30 minutes of taking it, much less semaglutide is absorbed. Studies on the effect of food found that even a small meal significantly lowers how much semaglutide reaches your circulation, compared with taking it on an empty stomach.1
One missed fasting window won't derail your treatment. But regularly taking the tablet with food, or breaking the 30-minute window, will make it less effective over time.
Forgot your tablet before breakfast? The general advice is to skip that day's dose and carry on as usual the next morning.
Do not take it with food, and do not take two doses to make up for it. If you're not sure, contact your prescriber.
Keep your tablets by your bed with a small glass of water, so taking one is the first thing you do.
Can you change what time you take Wegovy tablets?
The prescribing instructions say to take Wegovy tablets in the morning. For most people, that's when they've naturally fasted overnight, and it's the easiest time to fit in the 30-minute wait before breakfast.
If mornings are difficult for you, talk to your prescriber. What matters is an empty stomach and a consistent time each day. Morning is recommended because it's the most practical way to make sure your stomach is empty, not because it has any special effect.
Does the Wegovy tablet dose ever need adjusting?
For most people, once you reach 25mg, that's where you stay. It's the maintenance dose used in the OASIS 4 trial, which found an average of 13.6% body weight loss over 64 weeks.4
There are two situations where your dose may need adjusting.
Side effects are hard to manage. If nausea, vomiting or diarrhoea are severe or persistent on a particular dose, your prescriber may suggest pausing the increases and staying on that dose until things improve. That's a normal part of titration, not a sign treatment isn't working.
You have type 2 diabetes as well as obesity. The prescribing information notes that semaglutide levels from the tablet may be lower in people with type 2 diabetes, and absorption can vary more.2 Your prescriber will monitor you and may consider switching you to the injection if the tablet isn't working as expected.
How is the Wegovy tablet titration different from the injection?
The two forms of Wegovy have different titration schedules, and you can't simply swap between them mid-titration. The injection is weekly and goes up over 16 weeks from 0.25mg to 2.4mg. The tablet is daily and reaches 25mg over 90 days.
The biggest practical difference is the fasting rule. You can take the injection at any time of day, with or without food, with no limits on water or other medicines. The tablet needs a consistent morning routine built around the 30-minute fasting window.
This titration schedule is for people starting treatment from scratch. Already on the Wegovy injection or Mounjaro and moving across? Your starting dose will be different, so see our guide on how to switch from Wegovy injections to tablets.

When to seek further advice
Contact your prescriber if you:
- Experience severe or persistent nausea, vomiting, or abdominal pain that is not improving after dose escalation
- Develop symptoms of pancreatitis: severe abdominal pain radiating to the back, particularly following meals
- Notice that your blood glucose is consistently lower than expected if you have type 2 diabetes
- Are due to have surgery or a procedure requiring anaesthesia, and haven't yet informed the clinical team that you're taking a GLP-1 medication, as gastric emptying may be affected
- Have questions about other medications you take and whether the 30-minute window affects their absorption
A full list of side effects and drug interactions is available in the patient information leaflet supplied with your treatment.
Severe stomach pain spreading to your back? Get urgent medical help. It can be a sign of pancreatitis.
Frequently asked questions
How long does it take to reach the full dose of Wegovy semaglutide tablets?
Most people reach the maintenance dose of 25mg after 90 days. The titration starts at 1.5mg for the first 30 days, increases to 4mg for days 31–60, then 9mg for days 61–90, before moving to 25mg from day 91 onwards. Each step lasts 30 days to give your body time to adjust and to minimise side effects.
Can I take Wegovy tablets with coffee or tea?
No. Wegovy semaglutide tablets must be taken with water only - up to about 120ml. Coffee, tea, juice, or any other liquid other than water can disrupt the SNAC absorption mechanism that allows semaglutide to reach the bloodstream. You can drink your usual morning drinks after the 30-minute fasting window has passed.
What happens if I accidentally crush or chew a Wegovy tablet?
The tablets should be swallowed whole. Crushing, chewing, or dissolving them affects how the tablet erodes in the stomach, which is central to how SNAC creates the protective environment semaglutide needs to be absorbed. If you accidentally chew or crush a tablet, don't take a replacement dose. Just resume your normal routine the following morning and contact your prescriber if you're unsure.
Is the Wegovy semaglutide tablet dose the same for everyone?
The titration schedule is the same for all adults, and the recommended maintenance dose is 25mg once daily. There's no weight-based dosing. Unlike some other medications, the prescribed dose doesn't vary according to your starting weight or BMI. Your prescriber may recommend staying longer at an intermediate dose if side effects make escalation difficult.
Will Wegovy tablets interact with other medications I take?
Because Wegovy tablets slow gastric emptying, they can affect how quickly other oral medications are absorbed. The prescribing information advises particular care with medications that have a narrow therapeutic index (such as certain thyroid treatments or anticoagulants) and recommends waiting at least 30 minutes after taking the tablet before taking other oral medicines. Discuss your full medication list with your prescriber before starting treatment.
Are Wegovy semaglutide tablets available in the UK?
Yes. Wegovy semaglutide tablets were approved by the MHRA on 11 June 2026. Novo Nordisk expects them to be available by private prescription within weeks. The Wegovy injection also remains available at Pharmica for eligible patients now.
