Wegovy semaglutide tablets and Rybelsus are both oral semaglutide, both made by Novo Nordisk, and both use the same technology to help the medicine survive the digestive system. At first glance, they look remarkably similar.

But they are different medicines, licensed for different conditions, at doses that don't overlap, and with very different regulatory histories. Understanding the distinction matters particularly in the UK right now, where one of the two products is available and one is not.


What Is Semaglutide?

Semaglutide is a GLP-1 receptor agonist, which means it mimics a hormone called GLP-1 (glucagon-like peptide-1) that your body naturally releases after eating. GLP-1 signals to the brain that you've eaten, slows the rate at which food leaves the stomach, and triggers insulin release in response to blood sugar. By activating the same receptors, semaglutide reduces appetite, helps control blood sugar, and supports weight loss.

Novo Nordisk has developed semaglutide in several forms. The injection versions are Ozempic (licensed for type 2 diabetes) and Wegovy (licensed for weight management). The oral tablet versions are Rybelsus (licensed for type 2 diabetes) and Wegovy tablets (licensed for weight management). All four products contain the same active molecule. What differs is the dose, the licensed use, and in the case of the tablets, the formulation used to make oral absorption possible.


What Is Rybelsus?

Rybelsus is an oral semaglutide tablet licensed in the UK by the MHRA for adults with type 2 diabetes whose blood sugar is not well controlled with diet and exercise alone.1 It is not licensed for weight management and should not be prescribed or used for that purpose.

Rybelsus is available in three strengths and is started at a low dose that increases gradually. The current UK formulation, introduced in September 2025 with improved absorption, starts at 1.5mg once daily, increases to 4mg after one month, and can be increased to a maximum of 9mg once daily if more blood sugar control is needed.1 These doses are designed for blood sugar management, not for the higher level of GLP-1 activity needed to produce significant weight loss.

Like all semaglutide tablets, Rybelsus must be taken first thing in the morning on a completely empty stomach, with no more than 120ml of plain water. No food, drink other than water, or other oral medicines should be taken for at least 30 minutes afterwards. This waiting period is essential to how the medicine is absorbed.


What Are Wegovy Tablets?

Wegovy tablets contain oral semaglutide 25mg, which is significantly higher than the maximum Rybelsus dose, and are licensed specifically for weight management in adults with obesity (a BMI of 30 or above) or overweight (a BMI of 27 or above) with at least one weight-related health condition.

The tablet was approved by the FDA in December 2025 and launched in the United States in January 2026. Novo Nordisk submitted a regulatory application to the MHRA in late 2025, with a UK decision expected in late 2026. As of mid-2026, Wegovy tablets are not yet licensed or available in the UK. The currently available option for weight management with semaglutide in the UK is the Wegovy injection.

The dose titration for Wegovy tablets starts at 1.5mg once daily and increases every 30 days through 4mg and 9mg, before reaching the target maintenance dose of 25mg once daily. This gradual step-up is designed to give the digestive system time to adjust and reduce side effects during dose increases.2


How Are the Two Tablets Different?

Despite sharing an active ingredient, Wegovy tablets and Rybelsus differ in several important ways.

Licensed indication

This is the most important difference. Rybelsus is licensed for type 2 diabetes only. Wegovy tablets are licensed for weight management in people without diabetes who meet the BMI criteria. The two licences do not overlap. Using Rybelsus for weight loss without a type 2 diabetes diagnosis would be off-label use, outside the conditions its MHRA approval covers.

Dose range

Rybelsus goes up to a maximum of 9mg per day (current UK formulation). Wegovy tablets titrate up to 25mg per day. This difference exists because weight management needs a much higher level of GLP-1 receptor activity than blood sugar control. The 25mg dose is calibrated to achieve similar drug levels in the blood as the weekly Wegovy injection.2

Regulatory history and UK availability

Rybelsus received MHRA approval in 2020 and has been available in the UK since then. Wegovy tablets received FDA approval in December 2025 and are still pending MHRA review. As of mid-2026, only Rybelsus is licensed in the UK, and only for type 2 diabetes.

How absorption works

Both tablets use an ingredient called SNAC (salcaprozate sodium) to help semaglutide survive in the stomach. When the tablet dissolves, SNAC creates a concentrated zone of higher pH near the stomach lining. This temporarily disables the digestive enzyme pepsin and allows semaglutide to pass through the stomach wall and into the bloodstream.3 Without SNAC, stomach acid would break semaglutide down before it could be absorbed.

The reformulated Rybelsus introduced in September 2025 uses an updated version with better absorption, which is why the same treatment effect is now achieved at lower doses. Wegovy tablets use a larger amount of SNAC to support absorption of the higher 25mg dose. The fasting requirement for both tablets is for the same reason: food, other drinks, and other medicines interfere with how SNAC works.


Why Are the Doses So Different?

The gap between Rybelsus (up to 9mg) and Wegovy tablets (25mg) reflects what each medicine needs to do in the body.

For blood sugar control in type 2 diabetes, the main job of GLP-1 is to prompt insulin release after meals and provide some appetite reduction as a secondary effect. Clinical trials in the PIONEER programme showed that Rybelsus at 7-9mg produced meaningful reductions in HbA1c, a long-term measure of blood sugar control, in people with type 2 diabetes.4

For weight management, a much higher level of GLP-1 receptor activity is needed to consistently reduce appetite, slow how quickly food leaves the stomach, and change how the brain responds to food over time. The Wegovy injection achieves this at 2.4mg per week because it goes directly into the bloodstream, bypassing the digestive system. Oral semaglutide is absorbed less efficiently, even with SNAC, so a much larger dose is needed to reach similar drug levels. The 25mg oral dose was chosen specifically to match the blood levels achieved by the weekly injection.2

This is also why taking multiple Rybelsus tablets to try to replicate a weight loss effect would not only be unlikely to work but poses a real risk of overdose. The MHRA flagged this specific medication error risk during the September 2025 Rybelsus reformulation transition.5


What Does the Clinical Evidence Show?

The evidence base for the two products reflects their different purposes.

Rybelsus in type 2 diabetes

The PIONEER trials, which supported MHRA approval of Rybelsus, showed consistent reductions in HbA1c in people with type 2 diabetes. In the PIONEER 1 trial, the highest dose tested reduced HbA1c by 1.4 percentage points over 26 weeks, compared to 0.3 percentage points with placebo.4 Weight loss was measured as a secondary outcome: participants on the highest dose lost around 2.3kg more than those on placebo over the same period. This is a useful benefit for people with type 2 diabetes, but modest compared to what is seen in weight management trials.

Wegovy tablets in weight management

The phase 3 OASIS 4 trial tested oral semaglutide 25mg in 307 adults with obesity or overweight who did not have diabetes, over 64 weeks. Participants taking the 25mg tablet alongside lifestyle support lost an average of 16.6% of their body weight with full adherence, compared to 2.7% with placebo. When all participants were included regardless of whether they stayed on the full dose, average weight loss was 13.6% versus 2.2% with placebo. Around one in three people taking the 25mg tablet lost 20% or more of their body weight.6

A comparison of the OASIS 4 and STEP 1 trials found that oral and injectable semaglutide achieved broadly similar results for weight loss and heart health markers across comparable patient groups.7


Can Rybelsus Be Used for Weight Loss?

Rybelsus is not licensed for weight loss. In the UK, prescribing a medicine outside its licensed use is the individual prescriber's clinical responsibility and there is no approved pathway for using Rybelsus in weight management.

The evidence also suggests the doses available in Rybelsus would produce considerably less weight loss than Wegovy tablets. In the PIONEER 1 trial, the highest dose tested led to around 2.3kg more weight loss than placebo over 26 weeks in people with type 2 diabetes. That is a useful secondary benefit for that group, but much lower than the results seen with 25mg in the OASIS 4 trial.4

Some clinicians have used Rybelsus off-label for patients who have both type 2 diabetes and obesity, and who want to avoid injections. This is a case-by-case clinical decision, with the understanding that the 9mg maximum dose will produce a smaller weight loss response than Wegovy tablets at 25mg.

If you are looking to manage your weight, the licensed UK route is a supervised weight management programme using a medicine approved for that purpose. The Wegovy injection is currently available to eligible adults following a consultation.


Which Semaglutide Tablet Is Right for Me?

The two products serve different clinical needs and one does not substitute for the other.

If you have type 2 diabetes that is not well managed on your current medication, Rybelsus may be an option your GP or diabetes specialist considers as part of your treatment plan. The right choice depends on your existing medicines, blood sugar targets, kidney function, and other clinical factors.

If you want to manage your weight and do not have type 2 diabetes, Rybelsus is not the appropriate treatment. The licensed semaglutide option for weight management in the UK is Wegovy. Once Wegovy tablets receive MHRA approval, they will offer an option for people who prefer not to use an injection.

Neither product should be obtained or taken without a valid prescription from a registered UK prescriber. If you are unsure which treatment may suit you, speak to your GP or an online prescribing clinician.


When to Seek Medical Advice

Speak to your prescribing clinician if you:

  • Experience persistent or severe nausea, vomiting, or abdominal pain after starting either medicine
  • Have been taking Rybelsus for weight management without a prescription or outside its licensed use and have concerns
  • Notice symptoms of low blood sugar such as shakiness, sweating, or confusion, particularly if you also take insulin or a sulfonylurea
  • Have a personal or family history of medullary thyroid cancer or multiple endocrine neoplasia type 2, as both medicines are contraindicated in this group

Is Rybelsus the same as Wegovy tablets?

No. Both contain semaglutide as the active ingredient, but they are different medicines with different licences. Rybelsus is licensed in the UK for type 2 diabetes at doses up to 9mg. Wegovy tablets are licensed for weight management at a dose of 25mg. The two products are not interchangeable and one cannot be substituted for the other.

Can I take Rybelsus for weight loss?

Rybelsus is not licensed for weight loss in the UK. It is approved only for adults with type 2 diabetes whose blood sugar is not well controlled. Using it for weight management would be off-label use, outside its approved indication. If you want to manage your weight with semaglutide, the licensed UK option is the Wegovy injection.

Why is the Wegovy semaglutide tablet dose so much higher than Rybelsus?

Because each medicine is designed to do a different job at a different level of GLP-1 activity. Rybelsus doses up to 9mg are sufficient to improve blood sugar control in type 2 diabetes. Weight management requires a much higher level of GLP-1 receptor activity to consistently reduce appetite and produce meaningful weight loss. The 25mg Wegovy tablet dose was specifically chosen to achieve blood levels similar to those produced by the weekly Wegovy injection.

Are Wegovy tablets available in the UK?

Not yet. As of mid-2026, Wegovy tablets have been approved by the FDA in the US but are still awaiting MHRA review in the UK. A decision is expected in late 2026. Rybelsus is available in the UK, but only by prescription for type 2 diabetes. The Wegovy injection is the currently licensed semaglutide option for weight management in the UK.

Do Rybelsus and Wegovy tablets have the same side effects?

The side effect profiles are broadly similar because both products contain semaglutide. The most common effects are nausea, diarrhoea, vomiting, constipation, and abdominal discomfort, particularly when starting treatment or increasing the dose. Both tablets share the same contraindications, including a personal or family history of medullary thyroid cancer or multiple endocrine neoplasia type 2.

Do both tablets need to be taken on an empty stomach?

Yes. Both Rybelsus and Wegovy tablets must be taken first thing in the morning on a completely empty stomach, with no more than 120ml of plain water, and nothing else eaten or drunk for at least 30 minutes afterwards.
